I the Design survey window there is a drop-down menu where you add questions, For web surveys you can also choose Heading and Page break.
For web surveys you normally create one page per question. If you want several questions to be seen on the same page you add a Page break and move it upwards in the list, until it ends up after the relevant group of questions, where you want a new page to start.
If you choose to add a Heading, a window is opened where you enter your heading text using no more than 400 characters. You can also add a description of a maximum of 2,000 characters.
To create a heading on the same page as a specific question you add a heading and then move it up in the list until it ends up just above that question. In the screen shot below you see how Heading text and Description are seen above the question and its description.
